Please assist us by providing the following information at the time of your appointment:
- Your endodontic referral slip and any X-rays or digital images if applicable.
- A list of medications you are presently taking.
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

Your appointment includes appropriate diagnostic testing to verify the necessity for treatment.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves, and joints) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, bisphosphonates, etc.)
Sometimes a consultation appointment will be necessary to properly diagnose and establish a course of treatment. At this time, the treating doctor will explain the treatment options and, together with you, determine the appropriate treatment plan for your individual needs. A convenient time for you to begin treatment will then be scheduled.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or legal guardian.
Digital Images
If your dentist or physician has taken x-rays, you can bring them to our office, but they are not mandatory prior to scheduling your appointment. Initial digital images will be taken at our office.
